Behind-the-Scenes of an Appointment
Step 1: All new clients are required to book an initial visit. The first visit is 60-minutes. Upon making an appointment you will be required to complete, review, and sign the following forms including intake, policies/procedures, HIPPA, and release of information. Downloadable copies are provided below which you can fill and bring to your appointment.
Step 2: Based on the information presented on the intake form an agenda will be created to outline the consultation session. We will focus on the primary and two secondary concerns, review your history including symptoms, past or current approaches to dietary changes, and assessment of dietary pattern and knowledge.
Step 3: Individualization of education and recommendations will be provided. We will identify three interventions to choose from and develop SMART goals. SMART stands for Specific, Measurable, Achievable, Realistic, and Time sensitive.
Step 4: We will work towards the identified intervention(s) and goals and review any successes or barriers during the next scheduled follow-up. Follow-ups are 30-minutes long and will review any barriers or successes. Additionally we may make readjustments to goals and interventions, or add additional challenges.
