Behind-the-Scenes of an Appointment
Step 1: All new clients are required to book an initial visit. The first visit is 60-minutes. Upon making an appointment you will be required to complete, review, and sign the following forms including intake, policies/procedures, HIPPA, and release of information. Downloadable copies are provided below which you can fill and bring to your appointment.
Step 2: Based on the information presented on the intake form an agenda will be created to outline the consultation session. We will focus on the primary and two secondary concerns, review your history including symptoms, past or current approaches to dietary changes, and assessment of dietary pattern and knowledge.
Step 3: Individualization of education and recommendations will be provided. We will identify three interventions to choose from and develop SMART goals. SMART stands for Specific, Measurable, Achievable, Realistic, and Time sensitive.
Step 4: We will work towards the identified intervention(s) and goals and review any successes or barriers during the next scheduled follow-up. Follow-ups are 30-minutes long and will review any barriers or successes. Additionally we may make readjustments to goals and interventions, or add additional challenges.
Services and Booking
Payment on File Policy for No Calls / No Shows
Upon scheduling an appointment with Mackenthun’s Dietitian & Nutrition Services, we request you provide payment on file at time of booking.
The purpose of this policy is to hold your appointment slot and to hold you accountable to show up for the appointment; patients who fail to show for their appointments without notifying the provider in advance are costly to the practice and may prevent another patient who needs to be seen from getting an appointment. Mackenthun’s Dietitian & Nutrition Services reserves the right to charge a $50.00 fee of your designated service for no calls/no shows. Additionally, a convenience fee of 2.9% + $0.30 will be added onto the $50.00 transaction ($51.75) for online processing of payment through STRIPE.
Mackenthun’s Dietitian & Nutrition Services makes attempts to provide four reminders via text messaging and e-mail: At time of booking the appointment, 1 week before the appointment, 2 days before the appointment, and 4 hours before the appointment.
Charged fees for No Calls/No Shows are non-refundable regardless of event or occasion leading to the cancellation or missed appointment. It is up to you, the patient, to reschedule your appointment. If you contacted your provider 24 hours or more prior to your appointment for the need to reschedule, in this case, the fee will not be charged to the payment provided on file.
If you have missed 3 appointments without contacting your provider, Mackenthun’s Dietitian & Nutrition Services reserves the right to deny future appointments and in this case, Mackenthun’s Dietitian & Nutrition Services will provide you referrals/resources for other providers.
